Immersion is the best way. Find people you can converse with and do it until you are exhausted, which doesn't take all that long, ha. In high school, I took 3 years of Spanish, and spoke Spanish to my dog, who picked it up nicely, I must say. In my 20's, I used to talk only Spanish to my barbacks when I used to tend bar - they loved it, as it made their life easier. And everybody that didn't know what we were talking about hated it. Soooo... we started joking on everyone and laughing, which made it even more fun, and gave me more practice.
In my 30's and 40's I hired Salvadorans to help me with home renovations that I did regularly, and made a rule that we would speak English on odd numbered days and Spanish on even. It was funny because all of us complained how tired we were when we weren't on our native language day! I wanted to improve my skills, but also wanted to ensure that they had the tools to assimilate. They were good guys, and I wanted to help - leave it at that (UWS + universal "no" sign, please).

After all of that, I'm still not very good, IMO. My wife, who lived in Colombia with her Dad for several years when young, really humbles me. I have a good "ear" though, which is the greatest compliment I get - no me parece un gringo. But I engage latinos whenever I can, and watch Spanish TV (with English subtitles to cheat), and keep plugging away. But short of total immersion in a Spanish speaking country, be ready for a whole lot of frustration.
