What do we form tackle with? Shoulders. What is 6" away from shoulder? Head. It's going to happen and i don't think the vast majority has intent to "target". It's all done in the name of safety but you can only do so much. I mean, i have yet to see one of the 220# backs called for targeting when they lower their head and crash into a DB who is trying to tackle "heads up" and gets destroyed.
