I believe all of the European football leagues (not completely sure


about the Eastern European leagues) and all the South American leagues use some form of promotion/relegation. So from a geographical footprint that means the first (Russia), fifth (Brasil), and eighth (Argentina) largest countries in the world use promotion/relegation in some form. FWIW the US is the third or fourth largest country in the world (depending on how China is defined).

Also, there are plenty of examples throughout the world where a sports club (not just limited to football clubs) located in 1 country participates in another country's professional league. Even though the link is to a wikipedia article, the list seems to be pretty accurate.
