Who knows the ins and outs of our current medical system

Back story...just got this explanation of benefits from Anthem today, and haven't yet spoken to anyone at the doctor, lab, or Anthem.

Anyway, so my wife went to the dermatologist for a routine skin check. I've used this practice before with no issues. They've done a couple of extra things...removing a sketchy bit, biopsy, etc. Never a single cent that wasn't covered by insurance beyond a single $35 copay. While there, the wife saw a new doctor to the practice. The doctor recommended a "sticker biopsy", and did the thing with little explanation and certainly no discussion about a possible charge, etc. A few weeks later we get this explanation of benefits saying it was a non participating lab and we'll owe $1300.

Who do I call first, and what's my angle?
