We had a "Smith" machine on one of my ships, and...

...there was a kid lifting too much weight, and it smashed down on his face knocking out four of his front teeth. Guess he didn't have a spotter, nor did he engage the protective lockouts. There is additional "drop protection" that wasn't used, probably so he could go lower with the weight.

There were four small dents on the bottom of the bar which we thought were impressions from his teeth. Ouch!
