I want to say every semester there for a couple of years, then they disappeared. You didn't have to sign it but you assessed your score at the end to see if you were likely to become or were already an "alcoholic".

I did not drink much, if at all until my 4th and 5th years. But it didn't take much to be classified as an "alcoholic" IIRC.

Questions were things like:

1. I regularly drink more than 2 beers in one sitting
2. I use alcohol to relax
3. I have used "hair of the dog" to overcome a hangover
4. My performance and mood the following day have been affected by a prior night of drinking
5. I consume 10+ alcoholic drinks over a period of 7 days

So on and so forth. There were probably 20 questions like that and you scaled yourself on the old strongly agree to strongly disagree scale.

It was a big topic of conversation and ice breaker. "Hey, are you an alcoholic this semester?"
