Double plastic grocery bags inverted. Grab, pull, return the bag to normal


shape, and tie a not. Voila. I had to get steroids one year after wrapping around my arms to get full leverage to pull it out. Haven't had an issue since. The trick is to get it when it's small and still fits in the grocery bag.
