Probably the most misapplied rule in baseball and softball
And these new elbow guards players wear make it worse because players have little incentive to pull back into the box on inside pitches. It is so rarely applied correctly that when it is, such as earlier this year with a Carolina baseball batter at VT, everyone gets all out of shape. It's the same as offensive PI in football, happens all the time and isn't called.
The most egregious example was Barry Bonds. He hung that giant armor covered elbow guard over the inner part of the plate and didn't care if he got hit because it didn't hurt. That made everything in what is left of the strike zone an inside pitch for him, and he was a dead pull hitter. Allowed a 1 dimensional power hitter to be a record holder...well that and juice.
