What's really interesting is that he was shooting a single action revolver. You have to manually cock the hammer then pull the trigger for the hammer to fall. It's not like a semi-auto where you could have that one in a billion material failure that causes the firing pin to go forward without a trigger pull.

Maybe this is the universe correcting for the time where a bad guy pulled the trigger on Mike London and the gun didn't discharge.
