To the extent that cancer has an inherited cause I agree the rate attributed to those cancers has not increased. Our genome has not changed in 100 years. What has changed, in some places drastically, is the environment in which we live. Most of the things you mention in 5 are much more prevalent today. 100 years ago nobody died of cancer caused by exposure to Benzene, agent orange, asbestos, etc. Nobody died from cancer after watching an atom bomb go off in the desert. Those are new, 20th century phenomena. The cancer rate from these causes has gone from 0 to something, an increase.
