Certainly, it depends on the company.....or, more correctly, the size of the company but raises are not just given out without guidance. The are budgeted at a high level and flowed down based on an average.

I just went through the process last night of distributing by budget for merit increases to my team of 21. I had a fixed pot of money and guidance for increase % per person based on their performance rating. I have flexibility to distribute, but within guidelines that I cannot freely violate. If there are special situations, there is a process I can go through for "out of cycle" increases. If the justification is there, it won't take 12 months but it will take some time. Counter offering to someone who wants to leave can happen more urgently though.

This is BIG company policy. It is certainly done differently in smaller companies.
