I've experienced two different occasions when my insurance carrier tried to blame me/my wife for someone else's claim. The first instance was a woman with my wife's same first and last (married) name - this chick had an accident somewhere in the Pacific NW and it was showing up under my wife's driver history. I pointed out that we don't own the make/model of car involved and that neither of us have ever been to the state where the accident occurred. Thankfully that was about all it took to get it sorted out, but like you, it blew my mind that no one had ever checked driver's license numbers.

The other occasion was when my insurance company at the time (Allstate) sent me a cancellation notice for my homeowner's insurance soon after I purchased the policy saying that I had made too many prior claims and that they could no longer provide coverage for us. I was furious - first of all because I had never made a single homeowner's insurance "claim" in my life, and secondly because the claim they said I had made existed when I initially applied for the coverage, so how did I make it through the initial underwriting process if they had issues with "my" history? It turned out that they had confused me with my father (same first/last names, different middle names), and he hadn't even made a claim - he had simply called his insurer to inquire if some storm damage would be covered by his policy, but he never pursued a claim on it. Apparently insureamce companies flag you anytime you make an inquiry like that. I finally got someone with Allstate to tell me what had happened, and he was very apologetic and offered to reinstate my policy, but I said no thanks.
