There are a number of factors that make UNC and Duke intertwined.
Geography certainly helps. It's just a short drive down 15/501 to get to the other school. A lot of other universities have satellite campuses that are farther apart than that. Cultutrally and historically they have longstanding rivalries is sports, both football and basketball. Academically they have a joint scholarship program, where undergrad students are essentially students of both universities simultaneously, attending classes (and sports games) at both campuses in the same semester. And many of the students interact socially and have friends that attend the other school. There will routinely be cooperation on vising professors from one school's professor teaching a class at the other school. The administrations and faculties from both schools run in the same social circles, so many of them are friends or at least social acquaintances. And they share a lot of common alumni. You'd be surprised how many people I've met that went to one for undergrad and the other for grad school.
